Based on four staff files review, staff#3 did not have required Health Screening form on file which poses a potential health, safety or personal rights risk to persons in care.

Based on LPA's observation on 11/18/22 at 9:15 a.m. and 9:20 a.m. two children walked in the classroom alone, went to the bathroom alone and then joined teacher Asmik's class, the licensee did not comply with the section cited above and two children were observed without the supervision of a teacher from the time they entered the classroom, went to the bathroom and then joined the classroom which poses an immediate health, safety or personal rights risk to persons in care.

Based on LPA's observation on this date the licensee did not comply with the section cited above and LPA observed outdoor play structure area was full of dust, leaves, and spider webs which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on 10 children file review, 8 children did not have Physician Report LIC 701 on file. This poses a potential health, safety or personal right risk to the children in care.

Based of four staff files review, staff#1 and staff#4 did not have proof of immunization against measles, pertussis and influenza on file. This poses a potential health, safety or personal rights risk to persons in care.

1597.622 Employees or volunteers at family day care home; immunization requirements; records; exemptions (a) (1) Commencing September 1, 2016, a person shall not be employed or volunteer at a family day care home if he or she has not been immunized against influenza, pertussis, and measles. Each employee and volunteer shall receive an influenza vaccination between August 1 and December 1 of each year.

Based on LPA observation at 10:15 a.m., the licensee did not comply with the section cited above and LPA observed (2-3.5 year old ) classroom children store their jacket on top of each other on the bench outside the classroom (picture was taken) which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on LPA's observation on 11/18/22 the licensee did not comply with the section cited above and LPA observed in front outdoor playarea some parts of the grass are gone and the ground is not even which is not safe for children and can be a tripping hazards. Also there are some rose bushes and cactuses in the front outdoor playarea accessible to the children which poses/posed a potential health, safety or personal rights risk to persons in care.

On 03/01/2022 LPA Eivazian heard the first hour an half of recording from 11/10/2021. LPA heard staff#1 and staff#3 were talking in a proper way with other children, however LPA heard staff#1 and staff#3 talked inappropriate to child#1. This poses an immediate health/safety and personal right risk to the children in care.
