Based on staff file reviews, the licensee did not comply with the section cited above and staff#! did not have proof of immunization against TDAP and staff#3 did not have against immunization against MMR and TDAP which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per licensee, staff#1 and staff#3 will have required immunization by plan of correction due date, 06/30/23 and copies will be submitted to LPA Eivazian via email.
Based on staff file review, the licensee did not comply with the section cited above and licensees three assistants did not have TB Test clearance on file which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per licensee, she will submit TB test clearance for all three assistants by plan of correction due date, 06/30/23 via email to LPA Eivazian.
