MKRTCHYAN FAMILY CHILD CARE

Based on LPA's observations, the licensee did not comply with the section cited above, upon LPA's arrival to the facility on 05/08/24 at 12:40 PM, there were 7 children present in the home with licensee's assistant, all 7 children were under 6 years old, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Per licensee, she will disenroll one child from her daycare effective tomorrow and an updated facility roster will be submitted to LPA.

Based on facility file review and licensee, the licensee did not comply with the section cited above and licensee did not conduct fire drill with children at least every six months which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, she will conduct fire drill with children and will document the time and date.

Based on staff file review, the licensee did not comply with the section cited above, upon LPA's arrival to the facility licensee was not present in the home. LPA met with licensee's assistant, Satenik Martirosyan with seven children. Per licensee, Satenik Martirosyan did not have pediatric first and crp certificate on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, her assistant, Satenik Martirosyan will have current pediatric first aid and cpr on file by plan of correction date.

Based on staff file review the licensee did not comply with the section cited above and licensee's assistant, Satenik Martirosyan did not have proof of immunization against measles, pertussis and influenza on file. which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, her assistant will have required proof of immunization against measles, pertussis and influenza on file by plan of correction due date.
