Based on LPA's observation, the licensee did not comply with the section cited above, during this inspection 15 children were present, two being infants and 13 aging 2-5 year old and licensee was observed not be in licensed limitation and was out of ratio with three children which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Per licensee, she will disenroll three children by 10/20/25 and an updated facility roster will be sent to LPA by 10/20/25.
Based on LPA's observation, the licensee did not comply with the section cited above and upon LPA's arrival LPA observed child#1, infant was in the stroller during outdoor playtime and belts were fasten which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Per licensee, effective today she will only use the stroller for transportation and not for other purposes.
Based on children file review, the licensee did not comply with the section cited above and licensee did not complete infant nap chart which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per licensee, always an adult is supervising napping infants, and will complete infant nap chart for infants.
Based on staff files review the licensee did not comply with the section cited above in 2 out of 4 which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per licensee, staff#3 and staff#4 will have proof of immunization against measles, pertussis and influenza on file by 10/31/25.
Based on facility file review, the licensee did not comply with the section cited above and licensee did not have facility current roster which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per licensee, she will have current facility roster by 10/31/25.
Based on staff files review the licensee did not comply with the section cited above in 2 out of 4 which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per licensee, staff#3 and staff#4 will have TB test clearance on file by 10/31/25.
