Resolution: Per licensee, she will obtain child#1 immunization records by 06/27/25 and a copy will be submitted to LPA via email.
Based on six children files review the licensee did not comply with the section cited above in 2 out of 6 children, two current enrolled infants did not have Individual Infant Sleeping Plan on file which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per licensee, she will have Individual Infant Sleep Plan, LIC 9227 for two current enrolled infants by 06/27/25 and copies will be submitted to LPA via email.
Based on file review, the licensee did not comply with the section cited above. LPA has not conducted a fire drill with her enrolled daycare children which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per licensee, she will conduct a fire drill by plan of correction due date, 05/31/22 and licensee will submit a copy of disaster drill log to LPA.
