Based on observations , interview and records review ,the licensee did not comply with the section cited above. Assistant Jade Ricci did not have fingerprint clearance , which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee satted , assistant Jade will not be in the facility until fingerprint clearance is complete.

Based on staff file review, the licensee did not comply with the section cited above and licensee's assistants, Rebecca Gourley and Sandi Vega did not have TB Test clearance on file which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per licensee, her two assistants will have TB Test clearance by plan of correction due date and copies will be emailed to LPA.
