Based on facility 02/03/2023 sign in/out sheet review, the licensee did not comply with the section cited above. During this inspection 71 children were present and only 62 children were signed in which poses/posed a potential health, safety or personal rights risk to persons in care

Based on observations , records review and interview , the licensee did not comply with the section cited above. Three out of nine records were missing proof of mmr and Tdp, which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on four staff file review the licensee did not comply with the section cited above in 3 out of 4 staff reviewed files, staff#2, staff#3, and staff#4 were missing required mandated reporter training certificates which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on four staff file review, the licensee did not comply with the section cited above in 3 out of 4 staff reviewed files, staff#2, staff#3 and staff#4 were missing TB test clearance on their files which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on 10 children file review, the licensee did not comply with the section cited above in 2 out of 10 children reviewed files, child #3 and child #4 were missing physician reports on their files which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on 10 children files review, the licensee did not comply with the section cited above in 2 out of 10 children did not have Physical Report on file which poses/posed a potential health, safety or personal rights risk to persons in care.
