Based on staff file review, the licensee did not comply with the section cited above and licensee's assistant Lena Mikayelyan did not have proof of immunization against measles on file which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per licensee, her assistant, Lena Mikayelyan will have proof of immunization against Measles (MMR) on file by 04/25/25.
