Based on staff file review, the licensee did not comply with the section cited above and licensee's current three assistants, Arusyak Movsisyan, Marianna Shanoyan and Nune Baghdasaryan did not have proof of immunization against Pertussis (TDAP) on file which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per licensee, her three assistants will have proof of immunization against pertussis by 05/30/25 and copies will be submitted to LPA via email.
Based on licensee file review, the licensee did not comply with the section cited above and licensee's mandated reporter training certificate was expired as of 08/10/24 which poses/posed a potential health, safety or personal rights risk to persons in care.
