Based on LPA's observation from kitchen on 09/03/24, the licensee did not comply with the section cited above and LPA observed accessible knives on the kitchen cabinet and inside the drawer and cleaning compounds (Clorox) under the sink which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Per licensee, she will make the cleaning compounds and knives inaccessible to the children in the kitchen by plan of correction due date, 09/04/24.

Based on facility file review the licensee did not comply with the section cited above and licensee's son, Arman Khachatryan who has been residing at this address over a year, did not have criminal record clearance on file, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Per licensee, her son is a truck driver and once he is back during this week, he will complete his fingerprint clearance.

Based on licensee's file review, the licensee did not comply with the section cited above and she did not renew her required mandated reporter training, expired: 12/19/23, which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on staff file review, the licensee did not comply with the section cited above and licensee's assistant, Alvina Aleksanyan did not have proof of immunization against influenza, measles and pertussis on file, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, Alvina Aleksanyan will have proof of immunization against measles, pertussis and influenza by plan of correction due date.
