Based on record review, the licensee did not comply with the section cited above. During today's file review of Child 1's records, it was verified that that Child 1 was missing the required immunizations. After consulting with Child 1's parent, the Licensee confirmed that Child 1 has not received any of the required immunizations. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: The Licensee understands that Child 1 may no longer attend the day care until the child has received all required immunization. The Licensee will submit a copy of the Child 1's immunization records demonstrating that child is up to date with all required immunizations, at which time the child may resume attendance. Alternatively, the Licensee may submit a physician issued exemption specifying the medical reasons Child 1 is unable to receive the required immunizations. The submission of an exemption does not guarantee continued attendance, as a the Department must review the exemption and make final determination. The plan of correction must be submitted to CCL by the POC due date of 12/17/25.
