Based on LPA's observation, the licensee did not comply with the section cited above and per licensee she did not service her fire extinguisher as it is required which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per licensee, she will buy a new fire extinguisher and will send a copy of receipt to LPA via email by 08/15/25.
Based on tour for the facility , the licensee did not comply with the section cited above and licensee did not have Emergency Disaster Plan LIC 610A posted on the parents board which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per licensee, she will have the Emergency Disaster Plan LIC 610A posted on the parents and a picture will be submitted to LPA.
Based on licensee file review, the licensee did not comply with the section cited above and licensee did not have current mandated reporter training certificate on file which poses/posed a potential health, safety or personal rights risk to persons in care.
