DAVOIAN FAMILY CHILD CARE

Based on tour of the facility, the licensee did not comply with the section cited above and LPA observed child proof safety latch on the cabient under the sink in the kitchen and bathroom but LPA observed "Raid, Anut and Roach " spray in the cabient which consider as poison and it was not locked which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: During this inspection licensee removed the Raid sprays from daycare area. Per licensee she will ensure effective today not store and "Raid" in the daycare area.

Based on children file review, the licensee did not comply with the section cited above, per licensee, she did not complete infant nap chart for currently enrolled two infants which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, she will complete infant nap chart effective today and a copy will be submitted to LPA by 08/08/25.

Based on LPA 's observation, the licensee did not comply with the section cited above, during this inspection total of 13 children were present in the home, four being infants and one school age which poses an immediate health, safety or personal rights risk to persons in care. Licensee was observed to operate out of required license limitation with one infant.

Resolution: Per licensee, she will drop one or two children in order to meet required staffing ratio and capacity. An updated facility roster and a written declaration will be submitted to LPA Eivazian by 06/21/23.

Based on facility roster review, the licensee did not comply with the section cited above and did not have current facility roster which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, she will update her facility roster and a copy will be to LPA Eivazian by 06/30/23 via email.
