Based on observation, interview and record review, the licensee did not comply with the section cited above. During record review, LPA observed that the Mandated Reporter Training for the Licensee had expired on 12/29/2024 and Staff 1 was missing the Mandated Reporter Training. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee and Staff 1 shall complete the mandated reporter training and submit a copy of the certificate upon completion to CCL by the POC due date.
Based on observation, interview and record review, the licensee did not comply with the section cited above. Staff members 1 and 2 were missing the TDAP and MMR Vaccines. Volunteer 1 was missing the TDAP, MMR and TB Vaccines. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: The Licensee will ensure that all staff members and volunteers are immunized against influenza, pertussis, measles and TB and submit proof of correction to CCL by the POC due date.
Based on tour of the facility, the licensee did not comply with the section cited above and licensee did not have required Emergency Disaster Plan LIC 610A posted during this inspection which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per licensee, she will have the required Emergency Disaster Plan LIC 610A posted on the wall by plan of correction due date.
