ALRAHELI FAMILY CHILD CARE

Based on LPAs observations, the licensee did not comply with the section cited above LPAs observed inflatable pool was placed in the middle of backyard, in front of kitchen door which leads to the backyard. LPA Eivazian and licensee counted four steps from kitchen door to the inflatable pool. LPAs measured 5.5 inch water in the inflatable pool which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Per Licensee, she will remove the pool from backyard by plan of correction due date.

Based on LPAs observations the licensee did not comply with the section cited above, LPAs observed set of four knives on the top of kitchen cabinet by the sink and a mouth wash and hair products in the children bathroom which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: During this inspection licensee moved the knives from top of the kitchen cabinet in a high level cabinet out of reach of children. Per licensee, she will add a child safety latch to the bathroom cabinet.

Based on LPAs observation, the licensee did not comply with the section cited above and LPAs observed two rockers in the living room but did not observe any infants on the rocker which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee moved the two rockers from daycare area during this inspection. Per licensee, she will not use the rockers anymore.

Based on staff file review, the licensee did not comply with the section cited above and licensee's assistant, Zaina Hadi did not have proof of immunization against Pertussis, Measles and Influenza on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per Licensee, her assistant will have proof of immunization against Measles, Pertussis and Influenza on file.

Based on facility file review, the licensee did not comply with the section cited above and licensee did not have current facility roster which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, she will facility current roster available for review by 10/20/23.

Based on staff file review, the licensee did not comply with the section cited above and licensee's assistant, Zaine Hadi did not have TB test clearance on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, her assistant will have TB test clearance by 10/20/23.
