ALIKHANYAN FAMILY CHILD CARE

Based on observations, interview and records review ,, the licensee did not comply with the section cited above. LPA observed 2 out of 12 were missing immunizations records, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee satted she will obtain immunizations records from parents and will email to LPA by POC date .

Based on observations , records review and interview , the licensee did not comply with the section cited above. LPA observed 2 out of 12 children records were missing LIC 282 (affidavit) ,which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated required forms will be obtained during the pick up and proof will be sent to LPA by POC date .

Based on observations and records review ,the licensee did not comply with the section cited above . Licensees Mandated reporter certificate expired on 08/31/2024, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated , she will email completed certificate by POC date .

Based on staff file review, the licensee did not comply with the section cited above and did not have current Pediatric First aid and CPR certificate which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: During this inspection licensee and her husband had current Pediatric First Aid CPR that expires on 06/2023 but it was completed on line. Per licensee, they will take in person Pediatric First Aid and CPR course and current certificate will be submitted to LPA Eivazian via email by plan of correction due date 10/14/2022.
