Based on staff file review, the licensee did not comply with the section cited above and did not have current mandated reporter training on file which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per licensee she will complete required mandated reporter by plan of correction due date.
Based on LPA's observation, upon LPA's arrival to the facility at 11:00 AM LPA observed 8 children (2 to 5 years old) were present in the facility with licensee's assistant, Seda Ghazarayn. At 11:05 AM licensee arrived to the facility, licensee was operating out of her license limitation with two children, the licensee did not comply with the section cited above which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Per licensee, effective today, she will disenroll two children from her day-care.
