Based on licensee statement, the licensee did not comply with the section cited above and she did not complete infant nap chart for napping infants every 15 minutes which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per licensee, she will complete infant nap chart for all infants effective Monday, 02/09/26.
Based on LPA's observation on 02/06/26 at 11:50 AM, the licensee did not comply with the section cited above LPA observed daycare bedroom door, adjacent to the living room was closed, and observed an infant was napping in the crib which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per licensee, effective today, once an infant is napping, bedroom door will stay open and staff will complete nap chart every 15 minutes.
Based on staff files reviews, the licensee did not comply with the section cited above and licensee's present assistants did not have proof of immunization against Pertussis and Influenza on file which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per licensee her both assistants, Ani Tokhunts and Karmen Shakhbazyan will have proof of immunization against influenza and pertussis by 02/27/26.
Based on 6 children files review, the licensee did not comply with the section cited above in 2 out of 6 children did not have immunization on file which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per licensee, she will have child#1 and child#4 immunization record on file by 02/27/26.
