Based on observations, records review and interview ,the licensee did not comply with the section cited above. LPA did not observe current Mandated Reporter Training Certificate , which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will conduct the training and send the certifcate to LPA by POC date

Based on records review and interview, the licensee did not comply with the section cited above. LPA reviewed 5 records and records were missing LIC 995A ,which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will review children files and make sure files are complete and proof will be emailed to LPA by POC date.

Based on observations , records review and interview , licensee did not comply with the section cited above . LPA observed fire extinguisher in the kitchen, but licensee was not able to provide the receipt with the purchase date , which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licesee stated she will purchase new one and will notify LPA by POC date .

Based on records review and interview , the licensee did not comply with the section cited above. Licensee wa not able to find immunization records for 2 children files , which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: License stated , she wil obtain immunization records by POC date and will notify LPA.

Based on licensee file review, the licensee did not comply with the section cited above and did not have required mandated reporter training on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee she will complete required mandated reporter training by plan of correction due date 04/29/23 and send a copy to LPA Eivazian via email.
