Based on record review, the licensee did not comply with the section cited above. Child 5 immunization records need to be updated which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit updated immunization records for child 5 to CCLD by POC date.

Based on observation, the licensee did not comply with the section cited above. LPA did not observed a working carbon monoxide detector which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee replaced battery during visit. Deficiency cleared today.

Based on interview and record review, the licensee did not comply with the section cited above. LPA did not observe a Safe Sleep log for infants in care. Licensee stated she was told by GoKids log is only required for children one year or less which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated understands the regulation and she will begin to maintain a Safe Sleep log for infants 24 months and younger.

Based on interview, the licensee did not comply with the section cited above. Assistant stated they are not keeping a safe sleep log. which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee and assistant will begin to keep a Safe Sleep log for all infants 24 months and younger. They will submit a statement stating they have read Safe Sleep Regulations.

Based on record review, the licensee did not comply with the section cited above. LPA did not observe a current roster which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee's assistant completed roster during visit.

Based on observation, the licensee did not comply with the section cited above LPA observed a child sleeping in a crib with a barricade attached to it to avoid the child from getting out which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee and assistant understand child who was sleeping in the crib with the barricade is old enough to sleep on a cot and will not use the crib or barricade.

.3(a)(2) Alteration to Existing Building or Grounds. Prior to any alteration, the licensee shall notify the Department of the proposed addition of a room to the family child care home. This requirement was not met as evidenced by LPA observed a bathroom in the back right side of the yard. Maria stated it was put up about two years old. This poses a potential risk Health, Safety Personal Rights risk to children in care.

Resolution: Licensee was informed she must notify the Department prior to making alterations to the home and understands bathroom may not be used for day care children until a City Permit is obtained.

Based on record review, the licensee did not comply with the section cited above. Child 13 is missing LIC9227 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit LIC9227 for child 13 to CCLD by POC date.

Based on record review, the licensee did not comply with the section cited above. LPA observed a fire and disaster drill log which was last completed 03/12/2021 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will conduct a fire and disaster drill and submit to CCLD by POC date.

Based on observation, the licensee did not comply with the section cited above LPA observed a child sleeping in a crib with a barricade attached to it to avoid the child from getting out which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee removed barricade during visit and understands such items cannot be placed on crib.

Based on observation, the licensee did not comply with the section cited above. LPA observed an infant sleeping in a swing upon arrival. which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee and assistant understand children cannot sleep in swings and must be removed once they fall asleep. Licensee and assistant will read safe sleep regulations as part of their Plan of Correction.
