Based on observation, the licensee did not comply with the section cited above. Licensee was caring for four infants, six preschool age children and four school age children. Licensee was out of ratio which poses an immediate health, safety or personal rights risk to persons in care.

Based on observation, the licensee did not comply with the section cited above. LPA observed licensee's assistant completing Safe Sleep log for three infants in care. The log was last completed on 04/03/2024. One infant did not have a log which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit a statement stating she will maintain a Safe Sleep log for infants in care to CCLD bhy POC date.

Licensee stated they were placed there because they do not allow other children to sleep which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit a statement stating she understands high chairs are only for eating and children cannot be strapped to them for any other use to CCLD by POC date.
