Based on record review, the licensee did not comply with the section cited above. Child 10 is missing LIC627 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit LIC627 for child 10 to CCLD by POC date.

Based on observation and record review, the licensee did not comply with the section cited above. LPA observed six preschool children and six infants including two under one year old. Licensee is operation out of ratio which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit a statement how she will comply with regulation and maintain the correct ratio to CCLD by POC date.

Based on record review, the licensee did not comply with the section cited above. Child 7 immunization record need to be updated which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee requested immunization records for child when mom dropped child 7 off. Mother sent to licensee and printed them during visit. Deficiency cleared today.

Based on record review, the licensee did not comply with the section cited above Child 5 and 8 are missing LIC627 which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observation, the licensee did not comply with the section cited above. LPA observed a water table with water in it about halfway full which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee emptied water during visit. Deficiency cleared today.

Licensee is not maintaining a Safe Sleep log for infants in care for the duration of the time the child has been in care which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states she understands she must keep log for each individual infant in care for duration of time child is in care and for up to three years after child leaves facility.

Based on observation, the licensee did not comply with the section cited above. LPA observed two infants crying for a long period of time (one infant for approximately 20 minutes and another for 10 minutes) in separate play pens and being told to go to sleep in Spanish which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: LPA addressed the crying infants and licensee and her assistant picked up the infants in which they stopped crying. LPA explained regulation stating children can not be forced to sleep or stay in a designated sleep area.

Based on record review, the licensee did not comply with the section cited above. Licensee's assistant Lidia does not have immunization records against measles which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit immunization against measles for assistant Lidia to CCLD by POC date.

Based on record review, the licensee did not comply with the section cited above. Child 1 did not have immunization record on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee requested records for child 1 from parents and printed out during visit. Deficiency cleared today.
