Based on record review, the licensee did not comply with the section cited above. LPA observed a fire and disaster drill log was last completed on 12/22/2022 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will conduct a fire drill and submit a current log to CCLD by POC date.

Based on record review, the licensee did not comply with the section cited above. Licensee's husband did not have a Mandated Reporter certificate on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit a current Mandated Reporter certificate for husband to CCLD by POC date.

Based on record review, the licensee did not comply with the section cited above. LPA did not observe a current roster of the children which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit an updated roster to CCLD by POC date.

Based on interview, the licensee did not comply with the section cited above. Lissette De La Torre Banuelos does not have fingerprint clearance which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee understands Lissette may not remain in the home until clearance is obtained. Licensee understands that all adults living in the home must obtain clearance prior to residing in the home or having contact with the children in care.

Based on observation, the licensee did not comply with the section cited above. Licensee had six infants in care and out of ratio which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee must terminate care for two infants and inform LPA which infants she has terminated by POC date.

Based on observation, the licensee did not comply with the section cited above. LPA observed three infants asleep in play yards with blankets. One infant had a boppy pillow in play yard which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee removed items during visit. Deficiency cleared today.

Based on record review, the licensee did not comply with the section cited above. Assistant Norma has not completed Mandated Reporter Training which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit a Mandated Reporter Certificate for Norma to CCLD by POC date.

Based on record review, the licensee did not comply with the section cited above. LPA observed a child 5, 7 and 8 are missing from the roster of children which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit a complete roster to CCLD by POC date.

Based on record review, the licensee did not comply with the section cited above. Assistant Norma does not have immunization against pertussis, measles and influenza which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit immunizations for Norma to CCLD by POC date.

Based on record review, the licensee did not comply with the section cited above. Child 4, 5, 7, 8 and 11 are missing immunization records which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit immunization records for child 4, 5, 7, 8 and 11 to CCLD by POC date.

Based on observation, the licensee did not comply with the section cited above. LPA asked to see off limits bedrooms and found a lady, Lissette De La Torre Banuelos in an off limits bedroom with and infant. Infant was identified as a day care child and not Banuelos child which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee removed child from room. Licensee will submit a statement stating she understands she may not use off limit bedrooms for day care children.

Based on record review, the licensee did not comply with the section cited above. Child 11 is missing LIC9227 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit LIC9227 for child 11 to CCLD by POC date.

Based on interview, the licensee did not comply with the section cited above. Child 1, 2, 4, 7, 10 and 11 do not have a Safe Sleep log which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit a Safe Sleep log for infants in care for week of 09/22/2022-09/25/2022 to CCLD by POC date.
