Based on LPA's observation and interview with licensee, the licensee did not comply with the section cited above in that the infant enrolled and present today is not being logged for napping every 15 minutes. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will make sure she is checking infants (children under age 2) every 15 minutes while they are asleep and logging the napping. The licensee will submit proof of correction to the Department by 4/22/24

Based on LPA's observation and record review, the licensee did not comply with the section cited above in that the Mandated Reporter Training Certification has expired January 2024. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will complete the Mandated Reporter Training and submit proof of completion (the certificate) to the Department by 4/22/24. Licensee will remember to take the training before the expiration due date which is every 2 years.

Resolution: Licensee and adult daughter who is the assistant will completed the Mandated Reproter Training and submit the certificate to the Department by 1/18/2022.
