Based on LPA's record review, the licensee did not comply with the section cited above in that the licensee and the assistants do not have required Mandated Reporter Training which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee and employees will complete Mandated Reporter Training and submit to the Department by 5/20/24 via email.
Based on LPA's record review, the licensee did not comply with the section cited above in that there are no immunization records in the licensee's file or the assistants files. This poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will obtain copies of immunizations and TB test for the herself and staff. Licensee will submit to the Department by June 7, 2024.
