Based on LPA's observation, the licensee did not comply with the section cited above in that LPA observed two electrical outlets without the plate covered. LPA observed two holes in the wall. This is unsafe and poses a potential health, safety or personal rights risk to persons in care.
Resolution: The licensee will have the electrical outlets covered with protective plates and submit proof of correction to the Department by 6/30/25. The licensee will repair the holes in the wall and submit a picture of the repaired walls by 6/30/25.
Based on LPA Wheatley record review, the licensee did not comply with the section cited above in that the licensee has not been documenting the infants sleeping/napping every 15 minute on regular basis. LPA observed a notebook whereby the lciensee keeps notes and has noted some infant napping times. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: The licensee will ensure that she is following the Safe Sleep law and documenting infants sleeping and document the sleeping on a sleep log for the Department. Licensee will begin doing this immediately and submit a sample to the Department.
Based on LPA Wheatley observation and record review, the licensee did not comply with the section cited above in that the two assistants do not have proof of Mandated Reporter Training. This poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: The licensee will have the assistants complete the Mandated Reporter Training as soon as possible. Licensee will submit proof of the certification to the Department by 3/4/24 by email.

Resolution: Licensee will complete Mandated Reporter Training and submit a copy to Department by 12/16/22

LPA observed an off limit area being used for the infants. This was not approved by the Department. LPA did observe the room near the kitchen safe
