ARDON FAMILY CHILD CARE

Based on LPA's observation and record review, the licensee did not comply with the section cited above in that the Mandated Reporter Training for licensee and one assistant expired 8/2024. This is requierd for the licensee and all employees and poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee understands that the Mandated Reporter training expires every 2 years and will make sure she and all the staff complete the required training by 2/17/25.

Based on observation the licensee did not comply with the section cited above for not having carbon monoxide detector that is operating. This poses an immediate health, safety or personal rights risk to persons in care. Licensee did have the device however no working batteries on the premises.

Resolution: Licensee will purchase batteries for the carbon monoxide and submit proof of correction to the Department by 3/27/23. Licensee will ensure the device is checked often to make sure it is working at all times.

Based on (observation), the licensee did not comply with the section cited above in that several unsafe items such as Lysol and two burning candles were observed on the premises. This poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will remove all unsafe itemes from the reach of children and place up high or in locked cabinets to prevent any type of injury. Licensee will submit a plan of correction to the Department by 3/27/23.

Based on LPA's observation and record review, the licensee did not comply with the section cited above in that the licensee's adult daughter does not have a Criminal Record Clearance which poses an immediate health, safety or personal rights risk to persons in care. Licensee's daughter lives in the home and was cleared previously however the clearance changed. Licensee states she was unaware that her daughter was not cleared.

Resolution: Licensee understands how serious this is and will get her daughter re-fingerprinted immediately. Licensee states she will have her daughter reside some where else until she receives a Criminal Record Clearance and / or exemption. Licensee will submit proof to the Department a copy of the live scan form and the address where her daughter will be residing. Licensee will submit plan of correcton to the Department by 3/27/23.
