Based on observation and interview, the licensee did not comply with the section cited above in 1 out of 1 infant in care. LPA observed infant child #2 sleeping in a play yard with a pillow inside, which posed an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee immediately removed the pillow from the play yard as addressed by LPA during inspection.

Based on observation, interview, and record review, the licensee did not comply with the section cited above in 1 out of 1 infant in care. Licensee failed to provide LIC9227 for child #1 which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee states she will submit LIC 9227 for Child #1 to LPA by due date.

Based on interview and record review, the licensee did not comply with the section cited above in 2 out of 2 infants in care. Licensee confirmed Licensee did not maintian a Sleep log to document the 15-minute checks for the 2 infants, Child #1 and Child #2, in care, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee states Licensee will submit the Sleep logs for the infants in care to LPA by due date.

Based on observation and interview, the licensee did not comply with the section cited above, Licensee was observed to provide care for 9 children without an assistant from 11am-11:30am before the assistant arrived, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee immediately called assistant to come to the facility, assistant arrived 30 minutes after Licensee made the call.

Based on interview and record review, the licensee did not comply with the section cited above. Licensee failed to provide documetation of disater drills, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states Licensee will submit update Disater Drill log to LPA by due date.

Based on observation and interview, the licensee did not comply with the section cited above in 1 out of 2 infants in care. LPA observed Child #1 was sleeping on a chair instead of a crib/play yard. Licensee admitted Licensee did not move child#1 over to a play yard as soon as Child #1 fell asleep, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee immediately moved Child#1 out of the chair when LPA addressed the concern.

Based on interview and record review, the licensee did not comply with the section cited above in 1 out of 1 employee. Licensee failed to provide immunization record for assistant, Adult #1, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states Licensee will submit updated immunization records for assistant, Adult #1, to LPA by due date.

Based on interview and record review, the licensee did not comply with the section cited above in 4 out of [9 children in care. Licensee failed to provide children's files for 4 chidlren, Child #1, Child #2, Child #3 and Child #4, which poses a potential health, safety or personal rights risk to persons in care.

Based on interview and record review, the licensee did not comply with the section cited above. Licensee did not have an updated children roster for the children in care, which poses a potential health, safety or personal rights risk to persons in care.

Based on interview and record review, the licensee did not comply with the section cited above in 4 out of 9 children in care. Licensee failed to provide files for 4 children, Child #1, Child #2, Child #3, and Child #4, including the signed form LIC 995A, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states Licensee will submit LIC 995A for Child #1, Child #2, Child #3 and Child #4, to LPA by due date.

Based on interview and record review, the licenseeo did not comply with the section cited above in 4 out of 9 children in care. Licensee failed to provide LIC 282 Affidavit of Liability Insurance for Child #1, Child #2, Child #3, and Child #4, which poses potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states she will submit LIC 282 for Child#1, Child #2, Child #3 and Child #4, to LPA by due date.

Based on observation, interview, and record review, the licensee did not comply with the section cited above in 3 out of 3 adults. During inspection, LPAs observed 2 uncleared adults, Yen Nguyen and Thin Vo, were living in the home. Through interviews, licensee admitted that she hired an unclear staff, Ngoc, who worked in the month of October 2022 and November 2022, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: During inspection LPAs observed the two uncleared adults who lived in the home were asked to leave the property. Licensee states that Yen Nguyen and Thin Vo will no longer reside at this facility until they receive crimial record clearance.

Based on observation, interview and record review, the licensee did not comply with the section cited above in 1 out of 1 document. Licensee failed to provide current disaster drill log which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states that she will conduct a diaster drill and submit proof of disaster drill log to LPA Lucero by 12/19/2022.

Based on interview and record review, the licensee did not comply with the section cited above in 1 out of 1 certification. Licensee failed to provide a current CPR first aide certification which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states she will take CPR first aide training as soon as possible and submit to LPA Lucero no later than 01/05/2023.

Based on interview andrecord review, the licensee did not comply with the section cited above in 1 out of 1 employee. Licensee failed to provide staff file for adult 3 (A3), which posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states she will submit a letter of understanding of the regulations and a plan to ensure she will maintain employee records, to LPA Lucero by POC due date 12/19/2022.

Based on observation and interview, the licensee did not comply with the section cited above in 2 out of 2 adults who recently moved in the home, which poses a potential health, safety or personal rights risk to persons in care.
