SUNFLOWER PRESCHOOL CENTER & CHILD CARE

Based on record review and interview, Nina Nguyen worked at the facility without criminal clearance since April 26,2021 and PhuongLoan Le worked at the facility without criminal clearance since May3, 2021. The director failed to obtain a california clearance. This poses an immediate Health and Safety risk to the children in care.

Based on observation, interview, and record review, The licensee did not comply with the section cited above. At the time of LPA arrival observed none of the staff members present were able to demonstrate current Pediatric First Aid-CPR certification. At 1:40 pm Assistant Director arrived and assist the LPA, which posed a potential health, safety or personal rights risk to persons in care.

(b)(1)On or before March 30, 2018, a person who, ..licensed child care provider, administrator, ... mandated reporter training provided pursuant to paragraphs (2) and (3) of ... mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training.This requirement is not meet: Base on record review, S1, S2, S3 is missing Mandated reporter training. Also S4's Mandated reporter training was expired on 3/11/2020. This poses a potential Health and Safety risk to the children in care.

(c) The licensee shall notify the Department in writing of his/her intent prior to making any structural changes that reduce the total amount of indoor or outdoor activity space. Such structural changes shall include, but not be limited to, room additions.

(f) At least one staff member who is trained in pediatric cardiopulmonary resuscitation and pediatric first aid pursuant to Health and Safety Code Section 1596.866 shall be present when children are at the child care center or offsite for center activities.

(1) On or before March 30, 2018, a person who, on January 1, 2018, is a licensed child day care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training provided pursuant to paragraphs (2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training.

Personnel Records (a) The licensee shall ensure that personnel records are maintained on the licensee, administrator and each employee. Each personnel record shall contain the following information: (11) A health screening as specified in Section 101216(g).This requirement is not met: Based on record review, S2 is missing health screening. This poses a potential Health and Safety risk to the children in care.

Director stated this room was added without city permit and she did not know when it was added. Facility director did not notify the Department.
