Based on record review, the licensee did not comply with the section cited above. 2 out of 2 infants did not have sleep logs. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will provide proof of sleeps logs for 2 out of 2 infants to LPA by POC due date.

Based on observation, the licensee did not comply with the section cited above. LPA observed 2 cleaning product sprays in a cabinet underneath the sink in the children’s bathroom. Cabinet did not have any safety latches installed. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Assistant immediately removed cleaning products from under the sink. Licensee stated she will ensure cleaning products are stored away and inaccessible to children.

Based on observation, the licensee did not comply with the section cited above. The pool fence was approximately greater then 3 inches from the ground to the bottom of the fence. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will fix the pool fence to meet requirements by POC due date.

Based on observation, the licensee did not comply with the section cited above. LPA observed the fence gaps were greater than 4 inches. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will fix the pool fence to meet requirements by POC due date.

Based on observation, the licensee did not comply with the section cited above. LPA observed pool did not have either a pool cover installed or a pool alarm. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will purchase a pool alarm that meet requirements by POC due date.

Based on observation, the licensee did not comply with the section cited above. LPA observed pool did not have a life ring. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will purchase a life ring that meet requirements by POC due date.

Based on observation, the licensee did not comply with the section cited above. LPA observed pool did not have a rescue pole. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will purchase a rescue pole that meet requirements by POC due date.

Based on observation, the licensee did not comply with the section cited above. LPA observed pool did not have a daily inspection log. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will complete a daily inspecation log and submit proof to LPA by POC due date.

Based on record review, the licensee did not comply with the section cited above. 2 out of 3 staff did not have mandated reporter training certificates available. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will provide the mandated reporter certificate for 2 out of 3 staff to LPA by POC due date.

Based on record, the licensee did not comply with the section cited above. LPA observed staff #2 had a positive TB test. Licensee was unable to provide documentation for staff #2 's findings of positive TB test and Staff #3 did not have proof of tb test. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will provide proof of TB exam for staff #2 and #3 to LPA by POC due date.

Based on record review, the licensee did not comply with the section cited above. Staff #3 did not have documentation of the measles vaccine. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will provide proof of measles vaccine for staff #3 to LPA by POC due date.

Based on record review, the licensee did not comply with the section cited above. Child #1 (C1) did not have the measles vaccine. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will provide proof of measles vaccine for C1 to LPA by POC due date.

Based on interview and record review, the licensee did not comply with the section cited above in 1 out of 3 staff members, staff #1 did not have immunization record on file, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states she will submit current Mandated Reporter training certificate for Staff #1 to LPA by due date.

Based on interview and record review, the licensee did not comply with the section cited above in 1 out of 3 staff members, staff #1 did not have current mandated reporter training certificate on file, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states she will submit current Mandated Reporter training certificate for Staff #1 to LPA by due date.

Resolution: Licensee and assistant will complete the Mandated Reporter Training and provided proof to LPA Tran by 04/29/2022
