Based on interview and record review, the licensee did not comply with the section cited above, 2 out of 2 staff members did not have current Mandated reporter training certificates and Director confirmed that they did not have the current training certificates, which poses a potential health, safety or personal rights risk to persons in care.

Based on interview and record review, the licensee did not comply with the section cited above in 1 out of 2 staff memebers. Staff #1 did not have LIC 503 on file and Director confirmed that Staff #1 did not submit the LIC 503, which poses a potential health, safety or personal rights risk to persons in care.

(b) Each record shall contain information including, but not limited to, the following: (8) Medical assessment, including ambulatory status as specified in Section 101220, and the following health information:

(a) Prior to, or within 30 calendar days following the enrollment of a child, the licensee shall obtain a written medical assessment of the child. This medical assessment enables the licensee to assess whether the center can provide necessary health-related services to the child.

(g) The licensee shall document each child's immunizations and shall maintain such documentation in the center for as long as the child is enrolled.

(b) Each record shall contain information including, but not limited to, the following: (8) Medical assessment, including ambulatory status as specified in Section 101220, and the following health information:

(1) The center shall give each authorized representative a copy of the Personal Rights form (LIC 613A [9/96]).
