Based on interview and record review, the facility did not comply with the section cited above in 5 out of 6 staff members' mandated Reporter training expired which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observation, interview, and record review, the licensee did not comply with the section cited above in, During the personnel record, none of the staff members present has current EMSA-approved Pediatric CPR/First Aid. The Director stated she took the CPR/First Aid class in February 2023; however, the physical card is not in her Personnel Record. Her previous card was expired on 6/2/2023. which poses/posed a potential health, safety or personal rights risk to persons in care.

S1's current EMSA-approved Pediatric CPR/First Aid or American Heart Association card had expired on 2/2026. which poses/posed a potential health, safety or personal rights risk to persons in care.
