Resolution: Assistant, Angelica Rojas immediately, removed the pacifier clip from the child.
Based on observation and interview, the facility did not comply with the section cited above in having an infant on the sway swing sleeping, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Assistant Angelica Rojas stated the child had just fallen to sleep about 2 mins before LPA arrived to the facility. Assistant removed child to the play yard right away.
