Based on observation, records review, and interview, the licensee did not comply with the section cited above in 1 out of 1 fire extinguisher in the day care, which poses an immediate health, safety or personal rights risk to persons in care. The licensee stated that she has never serviced the fire extingyisher.
Resolution: The licensee stated she will buy a new fire extinguisher and send proof of correction to LPA by the due date above via email at archibaldo.silva@dss.ca.gov
Based on observation the licensee did not comply with the section cited above in 2 out of 2 infants in care, which poses an immediate health, safety or personal rights risk to persons in care. The licensee stated that she was not aware play yards had to be free of loose articles.
Resolution: The licensee removed all loose articles and objects from the play yards while the LPA was still in the facility. The licensee stated she will have an infant safe sleep regulations with teachers on Monday and provide proof via email to LPA by monday before the end of the day.
Based on observation, records review, and interview, the licensee did not comply with the section cited above in 1 out of 1 mandated reporter certificates, which poses/posed a potential health, safety or personal rights risk to persons in care. The licensee stated she was not awre she had to renew the mandated reporter training.
Resolution: The licensee stated that she will complete the mandated reporter training and provide proof of completion by the due date above via email to LPA at archibaldo.silva@dss.ca.gov
Based on observation, records review, and interview, the licensee did not comply with the section cited above in 1 out of 3 infants, which poses/posed a potential health, safety or personal rights risk to persons in care. The licensee stated that she did not have sleep logs for C3 who turned 2 years of age on 4/27/23.
Resolution: The licensee stated she will continue to carry out and document sleep logs for all infants in care and provide copies to LPA via email by the due date above.
