Based on interview and record review, the licensee did not comply with the section cited above the facility conducted a disaster drill on 1/7/25 which is more than 6 months from the requirements, this poses a potential health, safety or personal rights risk to persons in care.

Resolution: LIcensee stated they will conduct a disaster drill and will provide proof to LPA by due date of 8/28/25. Licensee stated they will provide a statement understanding of the regulation

Based on observation, interview, and record review, the licensee did not comply with the section cited above, an adult 18 and over living in the home and assisting licensee does no obtaint a fingerprint clearance or exemption, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will have the adult living in the home do fingerprint ASAP and will send LPA proof that lvescan was made by due date of 7/19/24.

Based on record review, the licensee did not comply with the section cited above in that licensee did not have a current diaster drill documented which poses a potential safety risk to persons in care.

Resolution: Per licensee will email a copy of the diaster drill log to LPA by POC due date.

Based on record review licensee did not comply with the section cited above in licensee did not have a proof of physically checkin gon the infants every 15 minutes which poses a potntial health and safety in care.

Resolution: Per licensee, will email copy of safe sleep log to LPA by POC due date

Based on ecord review the licensee did not comply with the section cited above in that Licensee and Assistant did not have a current Mandated Reporter Training certificate in file which poses a potential safety risk to persons in care.

Resolution: Per Licensee, will email LPA copies of Mandated Reporter Training Certification by POC due date.
