Based on record review and interview, the facility did not comply with the section cited above in 5 out of 5 staff files were reviewed. S1,S2, S3, S4, S5's Mandated reporter training certificate expired which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observation, interview and record review, the licensee did not comply with the section cited above. The facility could not provide a copy of the completed lead testing and LPA did not observe lead testing completed on the waterboard website. This poses a potential health, safety or personal rights risk to persons in care.
Based on LPA's interviews with 4 staff and observation of the physical plant it was determined C1 wandered away from the lower play yard under the supervision of S1 and was found alone on the ramp to classroom #2 by P1. C1 was left unattended for approximately 2-4 minutes. This imposes an immediate health and safety risk to children in care.

(1) A licensed child day care center, as defined in Section 1596.76, that is located in a building that was constructed before January 1, 2010, shall have its drinking water tested for lead contamination levels on or after January 1, 2020, but no later than January 1, 2023, and every five years after the date of the initial test.
(2) All prescription and nonprescription medications shall be maintained with the child's name and shall be dated.
Based on observation and record review, the licensee did not comply with the section cited above in LPA observed (1) expired medication which poses/posed a potential health, safety or personal rights risk to persons in care.
Based on interviews and record review, Staff 1 (S1) found Child 1 (C1) unsupervised in Room 1. C1 did not have care and supervision for approximately 5 minutes. This is an immediate risk to the health, safety risk to chidlren in care.
