LPA observation of sign in and sign out records. LPA observed six (6) authorized representatives of children who did not sign in and sign out a child on the correct date. This is a potential risk to the health, safety, or personal rights of children in care.

LPA observation of two staff files that did not contain a LIC 503. This is a potential risk to the health, safety or personal rights of children in care.

Based on observation, the licensee did not comply with the section cited above. LPA arrived at the daycare facility at 11:55 a.m., LPA was greeted by staff #1 (S1) who was supposed to be in Room #5 supervised the napping children. LPA observed that from 11:55 a.m. to 12:15 p.m., Staff #1 was assisting LPA and the children in Room #5 were not supervised. Licensee failed to arrangement appropriate supervision care in Room #5 while S1 assisted LPA. LPA observed that the children in Room #5 are ages between two to four year old. Due to the lack of supervision, there is an immediate health, safety, or personal rights risk to children in care. A civil penalty of $500 was assessment during this inspection.
