Based on observation, the licensee did not comply with the section cited above as stated in LIC809 and LIC809-Cs report, LPA observed various hazardous items that were accessible to children in care, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will replace all the safety latches that are broken in the kitchen, and restroom to ensure that children do not have access to "keep out of reach of children" items. Licensee stated she will make all other items, as stated in LIC809 and LIC809-C's inaccessible. Licensee stated that she will review regulation and write a statement on what they have learned and submitted it to the Department on POC due date.

Based on observation and record review, the licensee did not comply with the section cited above in that two (2) Infants were sleeping in a unlicensed room, door was closed, licensee was not conducting the 15 minute checks, licensee did not have current or past 15 minute checks, mattresses on play yards were not well fitted, and LPA observed a stuff animal inside the play yard with an infant, checks which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will review PIN 20-24-CCP RECENTLY APPROVED SAFE SLEEP REGULATIONS IN EFFECT and write a statement on what they have learned. Licensee will submit statement on POC due date to the Department.

Based on record review, the licensee did not comply with the section cited above in that Licensee and Assistant did not have a current Mandated Reporter Certificate on file, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she and her assistant will complete the Mandated Reporter Certificate for Child Care Providers AB1207 and submit it to the Department by POC due date.

Based on observation, the licensee did not comply with the section cited above in that LPA observed that licensee was using two areas of the home that were not licensed to be accessible to children in care, being used for children, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will review the regulation on reporting requirements and submit a statement on what they have learned to the Department by POC due date.

Based on observation, the licensee did not comply with the section cited above in that LPA observed the following in the hallway restroom: 15 packets of Clear Defense hand sanitizing wipes, 3 oz bottle of baby sun screen, 6 bottles of hand sanitizer, 1 tube of sunblock, a tube of toothpaste, and baby powder, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that they will ensure to make them items inaccessible and will write a statement and submit proof to the Department.

Based on record review, the licensee did not comply with the section cited above in that it was observed that licensee did not have a current fire drill on file, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that they will conduct a fire drill and submit proof to the Department by POC due date.

Based on observation, the licensee did not comply with the section cited above in that LPA observed that there were no 15 minute logs for infants in care, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that they will work on 15 minute logs for infants and submit them to the Department by POC due date.

Based on record review, the licensee did not comply with the section cited above in that Licensee and Assistant did not have proof on file of their MMR and TDAP immunization, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that they will submit proof of immunization for both themselves and Assistant by POC due date.

Based on observation, the licensee did not comply with the section cited above in that LPA observed that Licensee was caring for a total of 10 children on her own. LPA observed 4 infants and 5 preschool children, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that they will watch video HOW MANY CHILDREN CAN ATTEND A FAMILY CHILD CARE HOME? and write a statement on what they have learned. Licensee stated that they will also submit a statement on how they will ensure to keep facility within ratio.
