Based on record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. Licensing Program Analyst (LPA) observed that licensee and her staff were not consistently documenting the 15 minute safe sleep log for the two infants in care.

Resolution: Licensee agreed to submit 15 minute safe sleep logs for the two infants in care to the Community Care Licensing Office by the Plan of Correction due date, June 20, 2025.

Based on interview and record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. Licensing Program Analyst (LPA) observed that licensee was not documenting the 15 minute safe sleep checks for the infant in care.

Resolution: Licensee agrees to begin documenting the 15 minute safe sleep checks on the form provided and will submit proof to the Community Care Licensing office by the Plan of Correction due date, July 8, 2024.

Based on record review, the licensee did not comply with the section cited above in 2 staff did not have completed mandated reporter training, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will have staff 1 and staff 2 complete mandated reporter training and submit copies to CCL.

Based on record review, the licensee did not comply with the section cited above in 2 out of 2 staff did not have documentation of immunizations which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states that she will obtain documentation of required immunization and submit copies to CCL.

Based on record review, the licensee did not comply with the section cited above in 2 out of 2 staff did not have documentation of current CPR/First Aid, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will provide proof of paid enrollment into a apporved CPR course for either Staff 1 or Staff 2 to Community Care LIcensing by 8/15/22. .
