Licensing Program Analyst (LPA) observed that employees #3 and #5 were missing proof that they were vaccinated for pertussis and measles. Employee #3 was also missing proof of TB clearance.

Licensing Program Analyst (LPA) observed that three (3) out of the six (6) employees had completed the incorrect Mandated Reporter Training (General Training). LPA also observed that another employee had expired Mandated Reporter Training.

Licensing Program Analyst (LPA) observed that the floors inside of the bathroom in Room 2 had a strong odor of urine and that the floor around the toilet was starting to peel/lift up. In addition, LPA observed that the back wall and nearby floor boards in Room 2 were splattered with food stains. LPA observed that the rubber material on the stairs on the outdoors play structure was starting to crack, leaving sharp edges.

Isolation for Illness - (a) A center shall be equipped to isolate and care for any child who becomes ill during the day. Based on interviews, Facility Representative did not isolate ill children as it was reported that child #1 and child #2, who were showing symptoms of a communicable disease, were allowed to remain in the classroom with the rest of the children. This poses a potential risk to the health, safety, or personal rights of children.
