PEREZ, JACQUELINE FAMILY CHILD CARE

Based on LPA onservations and Licensee interview, the licensee did not comply with the section cited above. This is a two-story and stairs are not properly gated when children under age 5 years old are present which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will properly install a child safety gate around the stairs and make it inacessible when children under 5 are present and will submit pictures to LPA via email.

Based on observation, the licensee did not comply with the section cited above in that LPA observed that Licensee had three fountains in the front of the house. LPA observed water that was less than an inch collected in two fountains. LPA observed that Licensee's husband removed water that was collected in fountains, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licnesee stated that she ensures that water will be dumped out of fountains if they get filled by any water. Licensee stated that fountains will be filled with rocks or removed. Licensee stated that she will provide proof of corrections via photos to the Department by POC due date of 01/10/2024.

Based on interview and record review, the licensee did not comply with the section cited above. Licensee was unaware that provider needed to physically check on the infant every 15 minutes which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will physically check on sleeping infants every 15 minutes and keep documentation on a safe sleep log. Licensee will submit copy of safe sleep log showing two weeks of documentation to Fresno Community Care Licensing by 03/11/2022.

Based on record review, the licensee did not comply with the section cited above. Licensee's training on pediatric cardiopulmonary resuscitation and pediatric first aid expired on 05/04/2021 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will complete training on pediatric cardiopulmonary resuscitation and pediatric first aid and submit copy of certificate of completion to Fresno Community Care Licensing by 03/22/2022. If Licensee's appointment falls after 03/22/2022, Licensee will submit verification of appointment.

Based on record review, the licensee did not comply with the section cited above. Several children's files were missing immunization records which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will request immunization records for the children and submit copies to Fresno Community Care Licensing by 03/22/2022.

Based on record review, the licensee did not comply with the section cited above. Licensee completed mandated reporter training on 04/01/2019 and has yet to complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will complete mandated reporter training via www.mandatedreporterca.com and submit copy of certificate of completion to Fresno Community Care Licensing by 03/11/2022.

Based on observation, the licensee did not comply with the section cited above. Licensee does not have a fire extinguisher that meets standards established by the State Fire Marshall which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will obtain a fire extinguisher that meets standards established by the State Fire Marshall and submit verification to Fresno Community Care Licensing by 03/11/2022.
