Based on observation, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. Licensing Program Analyst (LPA) observed lotions, skin ointments, insect repellant on the kitchen table and on a low shelf in the kitchen. LPA observed Clorox wipes and cleaning spray under the bathroom sink. LPA observed a large bottle of mopping solution as well as bottles of shampoo on the floor of the walk in shower inside of the bathroom. LPA observed lotions, diaper rash medication inside of a basket on the floor in the bathroom, within reach of children. LPA observed stacked plastic bins and toys, which were a tipping hazard in the living room.
Resolution: Licensee agreed to make all lotions, ointments, shampoos, and cleaning supplies inaccessible to children by storing them in an out of reach area and/or adding a safety latch to the bathroom cabinet. Licensee agreed to also remove all of the stacked bins/toys from the living room by the Plan of Correction due date, 03/06/26.
Based on observation, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. Licensing Program Analyst (LPA) observed that the the pool fence did not completely surround the in-ground pool, as several windows around the house were only partially fenced and the exterior garage door and back gate allowed access to the pool. LPA observed that the pool gate was missing a key lockable device at least 60 inches from the ground. LPA observed that the pool did not have a safety pool cover or pool alarm. LPA observed that licensee did not have a life ring or rescue pole with body hook with the appropriate specificiations. LPA observed that licensee was not conducting a daily inspection of the pool nor was she maintaining a log.
Resolution: Licensee stated that she would add additional fencing as to prevent any access to the pool from all exterior doors and windows. She stated that she will add a lockable device to the pool gate no less than 60" from the ground. She stated that she will purchase a pool alarm, life ring, and rescue pole with body hook and will begin maintaining a daily log by the Plan of Correction due date, 03/20/26.
Based on observation and interview, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. Licensing Program Analyst (LPA) observed that licensee was not documenting the 15 minute safe sleep checks for the four (4) infants in care. LPA also observed blankets, pillows, and toys inside of the cribs where infants were sleeping. LPA observed the LIC 9227 form was missing for the infant in care that was under 1 year of age.
Resolution: Licensee stated that she will submit proof that she is documenting safe sleep checks for the four infants in care and will submit a copy the LIC 9227 for the infant in care under 1 year of age. Licensee agreed to review Safe Sleep regulations and submit a signed/dated statement that she read and understands the regulations by the Plan of Correction due date, 03/06/26.
Based on record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. Licensing Program Analyst (LPA) observed that licensee and Assistant #1 had only completed online training for Pediatric CPR/First Aid.
Resolution: Licensee stated that she and Assistant #1 will complete in person Pediatric CPR/First Aid training and will submit proof to the Community Care Licensing Office by the Plan of Correction due date, 03/06/26.
Based on LPA's observation of knives that were accessible to children in care. During the inspection, the children were eating and placed in high chairs or sitting on the child size table with licensee present. The licensee did not comply with the section cited above, which posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per licensee, she will utilize the safety locks on the drawers. During today's visit, licensee activated the magnetic safety lock. LPA will conduct a follow up visit to ensure the safety latches are being used.
Based on LPA's observation that the pool gate only self-latched sometimes depending upon the distance of the open gate. The door leading to pool area has two locking mechanisms. One that is on the top of the patio door way. Licensee did not comply with the section cited above, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Per licensee, she will tighten the pool gate spring to ensure that the pool self-latches at any distance. LPA will conduct a follow up visit.
Based on children #1, #2, #3 and #9 are missing the required form during reviewing of children's files. The licensee did not comply with the section cited which posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per licensee, she will have the missing forms completed by parents and place in children's files for review. LPA will conduct a follow up visit.
Based on interview with licensee, the 15 minute infant sleep is not documented. The licensee did not comply with the section cited above with all three infants in care. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: Per licensee, she will include documenting the 15 minute sleep log for infants. LPA will conduct a follow up visit.
