Based on record review, the infant in care did not have an Individual Sleeping Plan completed. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: The LPA gave the licensee a copy of the LIC 9227 form and the licensee agrees to have the parent complete the form by 8/19/22. A return visit will take place to clear this deficiency.
The licensee did not comply with the section cited above as the licensee had last conducted a fire drill on 11/2020. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: The LPA gave the licensee a fire drill log so that she would remember to conduct drills at least every six months. The licensee conducted a fire drill during the inspection.
Based on record review, the licensee did not comply with the section cited above as the infant sleeps in the day care room and is able to be checked during nap time. The licensee however was not logging the checks. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: The LPA gave the licensee a form that she can use to document 15 minute checks for infants. The licensee will have the document available for review by the due date of 8/30/22.
Based on record review, the licensee did not comply with the section cited above as the licensee and her assistant both had expired Mandated Reporter training's. They last completed them on 10/21/2019. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: The licensee and assistant agree to complete their Mandated Reporter training by the due date of 8/30/22 and have certificates available for review.
Based on record review, the licensee did not comply with the section cited above as two children did not have their immunizations transcribed and another child did not have proof of immunizations. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: The licensee agrees to get all children's immunizations updated and transcribed and have available for review by the due date of 8/30/22.
