MEDINA, CLAUDIA FAMILY CHILD CARE

During records review LPA Herrera did not observe immunization records for child #5 and incomplete records for child #3, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will ensure to obtain and submit records for child #5 and updated records for child #3 to CCLD via text/email by POC deadline.

During file review LPA Herrera did not observe an "Individual Sleeping Plan" (LIC 9227) on file for child #5, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will ensure to complete the LIC 9227 with child #5's parents and submit a copy of the LIC 9227 form to CCLD via text/email by POC deadline.

During records review LPA Herrera did not observe a 15 sleep minute log for child #5, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will ensure to documenet the 15 minute sleep log and submit a copy of the log to CCLD Fresno by 03/18/2025, via text/email.

Based on observation the licensee did not comply with the section cited above as LPA Herrera observed a water fountain/pond located in the front yard filled with water which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee Medina understands that the water feature can pose a safety risk to children in care. Licensee states she will make the water feature inoperable by turning off the water fountain and licensee will fill the water feature with rocks to prevent water from accumulating when it rains.Licensee will send photograph proof to Fresno RO by 09/26/2023.

Based on record review, the licensee did not comply with the section cited above in that, LPA observed that Licensee did not update their Mandated Reporter Certificate which poses/posed a potential health, safety or personal rights risk to persons in care. Licensee last completed their Mandated Reporter on 6/17/2021.

Resolution: Licensee stated that she will complete the Mandated Reporter AB1207 and submit proof to CCL by POC due date. Training can be taken at www.mandatedreporterca.com available in English and Spanish.

Based on record review, the licensee did not comply with the section cited above in Two infants in care with inconsistent documentation of safe sleep.which poses potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to submit evidence of current documentation of safe sleep for the two infants by 12/21/22 POC date.

Based on record review the licensee did not comply with the section cited above in all children enrolled in care were missing LIC 627 Consent for Medical treatment form which poses a potential health, safety or personal rights risk to persons in care.

Resolution: icensee agrees to submit evidence of copies of signed LIC627 Consent to treatment documentation for all daycare chidlren by 12/21/22 POC date.

Based onr ecord review, the licensee did not comply with the section cited above in 3 out of 5 children missing immunization records. which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Lincensee agrees to submit copy of updated immunization records for the 3 enrolled daycare children.
