Based on observation, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. Licensing Program Analyst (LPA) observed one toilet in the isolation bathroom that was not functioning, one faucet in the girl's bathroom that was not operable, a plastic cover over the toilet paper dispenser in the girl's bathroom was missing, and a paper towel dispenser in the preschool bathroom was missing a cover. LPA also observed that the colorful carpets in the School Age Room, Preschool Room #2, and Preschool Room #1 had staining and dirt marks.

Based on observation and interviews, the licensee did not comply with the section cited above. The facility is co-mingling school age, preschool children, and infant in beginning of the day and at the end of the day as the children are waiting to be picked up from the facility. This poses a potential health, safety, or personal rights risk to children in care.
