Upon record review, licensee and assistant were found to have mandated reporter expired on 4/6/2024 and 6/3/2024, respectively. Also, licensee and assistant did not have immunization records with current MMR and Dtap immunizations on file. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she and her husband will complete the necessary mandated reporter training course and receive blood labs to detect immunization for MMR and Dtap. Licensee will email the required documentaion and test results to LPA Tate.
Based on record review, the licensee did not comply with the section cited above. Licensee wass unable to produce a copy of assistant's AB1207. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: Per licensee, a copy of assistant's AB1207 proof of completion will be sent to Fresno North Community Care Licensing office before or by June 3, 2022
Based on record review, the licensee did not comply with the section cited above. Prior inspection on 11/20/2017, licensee was cited for same citation for licensee and assistant not proving proof of measles and pertussis vaccine. This poses a potential health, safety or personal rights risk to persons in care.
