GRAVES, MARCELLA FAMILY CHILD CARE

During today’s inspection, LPA Herrera observed cleaning compounds in the hallway closet, kitchen counter and kitchen sink cabinet. Cleaning compounds were observed to be in the hallway closet that was not locked with a latch, the kitchen sink cabinet did not have a safety lock and there were cleaning compounds in the kitchen's counter which posed a potential, health, safety or personal rights risk to persons in care.

Resolution: Licensee immediately removed the cleaning compounds and placed them in a locked closet where they were inaccessible to children. She also installed a safety latch in the kitchen sink's cabinet. The hallway closet was also immidiately closed with a latch. Licensee stated that she will ensure all the cleaning compounds are properly stored and made inaccessible. Licensee will also view the “Locks and Inaccessibility Requirements in Child Care” video and provide a written statement of what she learned by the plan of correction due date, 05/03/2024.

During record review LPA Herrera observed that licensee had not conducted a fire drill since 05/31/2022. The licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will conduct a fire drill by POC due date 05/07/2024 and write down the time the drill was conducted and how many children were present. Licensee will write a statement of understanding the importance of the fire drills and how she will make sure fire drills are conducted and documented accurately. Licensee will submit document to LPA by POC due date 05/07/2024. .

The licensee did not comply with the section cited above in that, during record review LPA did not observe a facility file for Child's Records for Child #2 and Child #3, and Child #1's file was incomplete which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will complete a facility file for Child's Records for Child #2 and Child #3. She will also ensure that Child #1's complete file is submitted to CCL by POC due date.

Base on observation - Staff 1 did not have evidence Mandated Reporter Training for Child Care Providers (AB 1207) which poses a potential risk to persons in care.

Resolution: POC: Licensee agrees to have Staff 1 complete the Mandated Reporter Training for Child Care Providers (AB 1207) and submit the certificate as evidence by POC date.

Based on observation, the licensee did not comply with the section cited above iOutdoor play area had many hazards accessible to children gardening tools, solar light fixtures taken apart, toys needing repair.which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to clean an organized the area, removes items not in use for daycare children, install mesh fencing to separate inaccessible areas by POC date

Based on record review, icensee did not comply with the section cited above Staff 1 did not provided evidence of Mandated Reporter Training AB 1207 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: LIcnesee will submit evidence of Manadated Reporter Trainng for Staff 1 to CCLD by POC Date.

Based on record review, the licensee did not comply with the section cited above Licensee and staff did not have evidence of immunizations Flu, MMR or Tdap. which posesa potential health, safety or personal rights risk to persons in care.

Resolution: LIcensee agrees to provide evidence of MMR, TDAP and Flu for herself and Staff 1. A statement declining flu is an option for evidence.by poc date

Based on record review, the licensee did not comply with the section cited above Licensee did not have records of 3 children in care. which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to mail in copies of records of 3 children in care. agrees to mail in copies of records of 3 children in care by kPOC Date .
