Based on record review, the licensee did not comply with the section cited above in LPA was informed by Staff #1 and Staff #3 that center had not conducted a disaster drill since the start of school, which poses/posed a potential health, safety or personal rights risk to persons in care.
Based on record review, the licensee did not comply with the section cited above in that LPA observed that Staff #1 and Staff #2 did not have a recent Mandated Reporter. LPA also observed that Staff #5 did not have their Physicians Report, which poses/posed a potential health, safety or personal rights risk to persons in care.

Children consist of Child #2, Child #3, Child #4, and Child #5, which poses/posed a potential health, safety or personal rights risk to persons in care.
Based on interview, the licensee did not comply with the section cited above in that the classroom AC units were not working properly and that the School had provided Classroom 1 and Classroom 2 with space heaters to warm up the classrooms. LPA was informed that AC unit will be replaced, which poses/posed a potential health, safety or personal rights risk to persons in care.

LPA also observed some ants on the cubbies near the front entrance in Room 1 and around the outside of the door in Room 2. Lastly, LPA observed a strong odor of urine coming from the bathroom in Room 1.

California Lead Action Level at Child Care Centers (a) California’s Action Level for lead in water at Child Care Centers is 5 ppb. (b) Testing results with fractional ppb readings of 0.5 ppb or greater shall be rounded up to the nearest whole number, before comparing to the Action Level. (1) A result with values of 5.5 ppb or greater shall be deemed an Action Level Exceedance. (c) If testing indicates an Action Level Exceedance at any water outlet, the water from that outlet is deemed not safe to drink and an immediate response pursuant to section 101704 shall be required. This requirement was not met evidence by: Based on record review provided by licensee indicating (ALE) of 17.7 ppb in outlet B and an (ALE) of 9.56 ppb in outlet C impacting K-2. These results indicate out of compliance over (ALE) 5.5 ppb, which poses a potential health, safety or personal rights risk to persons in care.
(a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors.
