Based on record review, the licensee did not comply with the section cited above due to staff #3 not having a completed LIC 503 on file with a TB screening results, which poses a potential health, safety or personal rights risk to children in care.
Based on observation, the licensee did not comply with the section cited above due to the playstructure missing the safety rail (slide hood) that is place above the slide. Per staff the safety rail has been missing for approximately a year, which poses a potential health, safety or personal rights risk to persons in care.
Based on record review, the licensee did not comply with the section cited above due to the children's emergency medical consent form missing parental signatures for 2 out 4 children files reviewed. Furthermore, LPAs reviewed the additional emergency medical consent forms and observed 3 other children's form missing parental signatures, which poses a potential health and safety or personal rights risks to children in care.

Drinking water - from a noncontaminating fixture or container shall be readily available both indoors and in the outdoor activity area. Children shall be free to drink as they wish. Drinking water fountain is covered with vines, trash in the basin and is not working. Water is not readily available outdoors for children in care. LPA observation and interviews reflect this requirement is not being met. This poses an potential risk of personal rights, health and safety to children in care.

Personal Rights. To be accorded safe, healthful and comfortable accommodations, furnishings and equipment to meet his/her needs. The climbing structure has broken components, children are being biten by ants and the exerior wall of building has wood rot/fungi. There are reports of termites. LPA observation and interviews reflect this requirement is not being met, This poses an immediate risk to personal rights, health and safety to children in care.
