Based on file review, the teachers file was not complete and the sub did not have a file available. This poses a potential health, safety or personal rights risk to persons in care.

During inspection, the entire classroom including all staff left to the cafeteria for lunch. The staff left behind one child. LPA was able to call for one of the teacher's to come back to get the child. The licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

During review of the children's file, 0 out of the 5 children reviewed at the required licensing form LIC 627 Consent for Medical Treatment on file. Child #1, Child #3, and Child #4 were missing all licensing forms, LIC 613A, LIC 700, LIC 701, LIC 995. Child #2 was missing LIC 700, and Child #5 was missing LIC 700 and LIC 701. The licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.
